Health sharing

Health sharing, with the fine print first.

Sharing programs can cost less each month than insurance. They are not insurance, they can limit pre-existing conditions for years, and they don’t have to pay any bill. Here is how they work, and every program we track, ranked for your answers without payment data.

  • Prices dated and sourced
  • Legal history shown
  • Ranked without payment data

How it works

A membership, not a policy.

Members pay a monthly amount. When someone has a big bill, the program decides under its own guidelines whether the bill is shared. That is the whole trade-off: a lower price for less certainty.

You pay first

Most programs have an amount you pay on each need (often $500 to $5,000) before sharing starts. It works like a deductible, but per need.

Conditions wait

A condition you already have is usually shared only after one to five years, and some are never shared. Insurance covers them now.

No legal promise

State insurance departments don’t regulate sharing, and no program has to pay a bill. A few have faced regulators and courts; we show that history.

It may suit you if

  • You are healthy and want a lower monthly cost
  • You likely won’t qualify for help paying Marketplace premiums
  • You can cover the amount you pay first on each need

Look at the Marketplace first if

  • You or someone in your home has a health condition
  • You are pregnant or planning to be
  • Your income may qualify you for premium help
  • You are 65 or older: see Medicare
  • You live in CA, DC, MA, NJ or RI (state penalty)

Health care sharing programs are not insurance. They are not regulated by state insurance departments, do not have to pay any medical bill, are not subject to ACA protections (essential health benefits, pre-existing condition coverage, annual and lifetime limit rules), and members do not get ACA premium tax credits for them. Members remain responsible for their own bills.

California, DC, Massachusetts, New Jersey and Rhode Island have their own coverage requirement, and you may owe a state tax penalty without qualifying coverage. Only health care sharing ministries operating since 1999 can qualify for an exemption; medical cost-sharing programs do not.

Know the labels

Five kinds of company, one word: sharing.

Companies use similar names for very different things. The kind tells you what rules apply.

Health care sharing ministry (HCSM)
A faith-based nonprofit that has shared medical bills continuously since before 2000. Only these can qualify for the state-mandate exemption. Members agree to a statement of faith or standards.
Medical cost sharing
The same idea without the ministry status, often with no faith test. These programs do not qualify for mandate exemptions, and regulators have treated some as unlicensed insurance.
Crowdfunding
Members fund each other’s large bills through an app. Funding is not guaranteed and pre-existing conditions are not fundable at first.
Membership services
A company that bundles virtual care and perks with access to someone else’s sharing program. Read whose guidelines govern the sharing.
Direct primary care
A flat monthly fee to a primary-care practice. It is not catastrophic protection; pair it with a plan that covers hospital bills.

Questions

Good questions, short answers.

Is health sharing the same as health insurance?

No.

Can a health-sharing program refuse to pay a bill?

Yes. Programs share bills under their own guidelines, and none of them is legally required to pay a given bill. Read the guidelines for limits, waiting periods and services that are never shared before you join.

Do health-sharing programs cover pre-existing conditions?

Usually only after a wait. Many programs share little or nothing for a condition in the first year and phase in over two to five years. For example, Zion HealthShare shares $0 in year one and up to $125,000 per year from year four, and Medi-Share shares up to $100,000 a year after 36 months. ACA Marketplace plans cover pre-existing conditions from day one.

Source: Each program’s published guidelines, checked 2026-10-15

Will I owe a state tax penalty if I join a sharing program?

California, DC, Massachusetts, New Jersey and Rhode Island have their own coverage requirement, and you may owe a state tax penalty without qualifying coverage. Only health care sharing ministries operating since 1999 can qualify for an exemption; medical cost-sharing programs do not.

How do you rank the programs, and does anyone pay you?

Our formula uses only the answers you give and our dated facts: 35% fit to your answers, 20% expected cost, 20% protection strength, 15% track record and 10% transparency. Fees are never an input. Today we have no signed referral agreement with any company on this page, so every card is labelled Listed. Our founder is the Chief Technology Officer of MPB Health, so MPB is left out of the ranking and shown separately, with that disclosure. MPB Health is not insurance; its cost sharing is administered through Zion HealthShare (see the disclosure next to it).

When can I join?

Sharing programs take new members all year. ACA Marketplace open enrollment for 2027 coverage runs Nov 1, 2026 to Jan 15, 2027 on HealthCare.gov; enroll by Dec 15, 2026 for coverage that starts January 1. Some state exchanges set other dates.

I’m 65 or older. Should I look at health sharing?

No. Medicare is your coverage path, and we don’t show health-sharing programs or referral links to Medicare visitors. Our Medicare guide is at olyronmedicare.com.
